Hi, Mark. I looked on the club website and found the Hall of Fame page, but there's no link to the nomination form. I thought the forms were included in the club magazine but I just skimmed through it (and the table of contents) and didn't see any mention of it.

Can you post one here and also over on the CCA message boards under "Club Business", please?

If you'll email me a PDF, I'll send it over to Mike and have him add a link to it on the HOF page itself, which is the perfect place for people to find it.
