The ceramic mold chip, named the "cards mold", is available as a blank from a number of places. From China, and also from resellers like this one:

The buyer can then provide artwork for a finished chip of any design.

As far as I know, this particular card-mold ceramic chip is not approved for use by Nevada Gaming Control.
