I think Jim is right that the unusual mold is an error. Perhaps a greek key pattern that was overstamped / double-struck on that one side?

It appears that the chip was made by a company called Claysmith Gaming, though it seems that they may be out of business now. Their ".com" page doesn't show, though they did have a facebook page.

Here's a sales page on Amazon showing their chips with the 12-stripe insert option on the greek key mold, though their narrative says they call this mold "Meander" instead.

Note in the upper left corner, you'll see your green chip with the same 12-stripe color as yours. And the light blue chip also has the same 12-stripe colors, though it is hard to see peeking out from behind the black box narrative.

The inlay on your chip seems to be a custom-created sticker for "Cincinnati Festival Alliance".
