This is my second try at posting a chip for ID purposes on here, hope this one works. :)

I know it's a Planet Hollywood chip, I'm assuming a NCV, being a rookie I know Caesars has a Planet Hollywood, but for all I know this could have come from somewhere else? Anyone know? How about TCR# if it is a Nevada chip?
