The 40 chip is from the Monte Carlo Casino in Monaco. The hole is most likely from being made into jewelry. There was necklaces made of old Monte Carlo casino chips. I found one of the necklaces about 15 years ago at a flea market, of course I took all the chips and jetons off the necklace.
