There is now a flat rate international box that will hold several 100 chips for $12.95.
Still use the same green form you are showing.
Be careful how much value you put on it. Some countries have customs fees for the person recieving the package if over certain limits.
I always put $15 on them.

You are correct on smaller amounts like 1 to 10 or so chips.
