The same factors that can be used to straighten them out: heat, moisture, bending force.

Chips that are dug out of concrete are usually warped as well as surface damaged by the moisture and chemicals in the cement. In severe cases, there are cracks that develop between the edge inserts and the body of the chip.

Old plaques will warp all by themselves as chemical changes cause stresses in the materials, which are not uniform since most old plaques have inserts or "lunettes".
