Dale did list this design in the supplement dated 2001. He has different codes for the chip you show and its mirror image. However, notice that all that was done in manufacture was to flip the inlay over before the process was completed. I have the scotty dog inlaid poker chip with the same kinds of mirror images. I think they are just errors or maker's defects, not different chips.
