In my collection I have:
1st issue - no UV code
1st issue - small H&C uv code
5th issue "Live the Legend" - large H&C and "00" (really weak so this may not be correct)
5th issue "Live the Legend" - large H&C and "02"
6th issue "1979-2004" - large H&C and "03"
6th issue "1979-2004" - large H&C and "04"

I think the 5th issue has a "99", "01" and possibly an "03", and the 6th issue maybe an "05". I've been buying a lot of these trying to fill my gaps but it would be nice to know exactly what the gaps are.
