Steve, I believe the Horseshoe Casino chip you show is a fantasy chip also. The Gardena club was always named "The Horseshoe Club" not "Horseshoe Casino". I bought a set of these in various denominations from the Paulson retail store several years ago, and after the club in Gardena had closed. It's possible that they were planning to change their name, but closed abruptly instead. However, I think it's more likely that these were just a Paulson creation for sale as fantasy chips.
As to the Players chip from Pullman, WA, I can't explain the use of the non-unique set of inserts, unless it was just a mistake.
