I use the 1/2"x5/16", and I only use 2 per holder.
I have the fold on the left and use 1 label on top of the holder, and one at the bottom. That way I can easily slide the chip out to look at it or take a picture of it. We also have a Brother Label machine that I use to put the tcr # on the holder.
