Some info on the H&C insert style you're interested in --
Currently made by Paulson, which is part of GPI: Gaming Partners International. Their website has a 24-page pdf showing various chip styles & colors. See page 7 of the pdf for insert style # 31, which is the one you're interested in.

Previously, Paulson has also labeled this insert style as 12-1/8apl. Decoding this label: there are 12 inserts (total) that are each 1/8th inch wide. The "pl" refers to the sets of 3 inserts being "parallel" to each other, rather than radially-oriented, as in style #19. At least that was the explanation shared with me some years ago.
