The ones I mentioned did not have LTD and number minted on the chip.
No re-issues with LTD and number minted, comes to mind.

I think I have a spread sheet with all the re-issues somewhere.
They include;
Boomtown
Nuggett in Laughlin
Boulder Station or Palace Station, forgot which one.
Nevada Palace
Mandalay Bay has openly stated they will re-issue Millennium chips if the ones they did, all go away.

Probably others.
