Many of your questions can be answered that the collectors own personal preference will control what is collected.
Some people value a card with out a name higher than one with a name. I in my collection do not care: I will collect one with my name, or one with any name, or one with no name.
Imprints/and/colors vs embossed is mostly a matter what machines the casino is using and often varies with in the casino at different issue stations.
The embossed type are a left over technology from the days when the cards were put in an a machine to print the embossed data on paper (mostly for old time credit cards) no longer used for that.
I do not rank them as a diff. card type.
