I picked up the chip in the scan (sorry it isn't bigger but I'm using a scanner I'm not used to using) at a coin shop in Sydney, Australia. The mold seems to be clubs and triangles. I searched the archives and find some posts about an Elite Club in Wells, Nevada. Did I get lucky and find a Nevada chip?
