You are correct about the Lady Luck! I guess they never did get Gaming International made Win Cards NCV chips. The Win Cards brochure was customized to the Lady Luck, to include a photo image of Lady Luck chips.

Regarding the Four Queens chips.... I have included pictures of both sets that I got and the slot cards I used to get them. At that time you were limited to a one time transaction and you had to tell them if you wanted fifteen 1 NCVs or three 5 NCVs.

I got the fifteen 1 NCVs and had my wife sign-up and get the three 5 NCVs.

So yes, I am certain that the Four Queens 1 and 5 NCVs were used for the Win Cards promotion when it first started.
