I have a Bally's card I got in Las vegas several years ago that is a little different that your samples. It looks the most like your middle one above, brown with the mutli-color striped "MVP"s running along the lower edge.

Differences: Main change was the lack of the bush/tree (whatever) on my card. it is just blank above the Bally's name. Mine seems darker brown, but that may be the scanning on your part, if you lightened it for clarity. On the reverse, text is jumbled around a mite, it doesn't list all the locations, just a Las vegas number. It has "SC24" in the upper left corner.
