If I pay extra for insurance, I assume that there will be evidence of this on the package, otherwise the seller should tell me he is "self insured" thru an insurance company. If he is "self insured" and there will be a charge for service then this should be noted in the ebay listing. I buy from Andy Hughes and he is "self insured" and he will NOT let me pay for it.
