That would be the 204 Club in Hallandale, FL, about 1/2 hour from me. I emailed Mark Lighterman and he did not feel they were from the Hallandale Club. He didn't think the wave crest mold was used there.

Allen, the 204 Club was my first thought. I have no way of making any firm attribution at this time, however, that is the only 204 Club I am aware of.
