It's called the Triangles and Clubs mold or Tri-Club. Eisenstadt's site says it was used until "20 years ago" whenever that means. Pre-2000, I'm sure. I would contact David Spragg for information. There was also someone on the board who was researching years of mold use and years of manufacturers/distributors. You could do a search to figure out who it is.

I have no idea about the design on the reverse. It appears to me, though, that there is nothing beneath the spiral hot stamp design, so I'm not sure it's a cancellation.
