Let me add this to my post: I'm quite certain all 5 were used at the Chez Paree. Gene Trimble in his "Illegal of the Day" article about the place, shows pictures of chip #1 (white cord) and #2 (chocolate hub) in the article. And the person who sold them to me knows much about the place and says they are from the club, originally got the chips all together.

By the way, I got just 4 of this chip, which I am holding onto for the moment (not for sale yet) -- white hub mold:
