How to see the entire "Old Poker Chips" file

I tried last week to look at the link I had to the reference on antique poker chips that I had. It's at:

www.oldpokerchips.com

I now only see the cover page and no way to link to the sections of the PDF file that show the various categories of chips. I have tried two different browsers from my PC running Windows 7. I do have the latest Adobe Reader (I think).

If I guess at the name of a particular section, like

http://www.antiquepokerchips.com/2011/Products.pdf

I do see that section. I'd like to see the page of the file that had all the links available. How do I do that?

From the name, the files seem to be on Rich Hanover's web site. He doesn't know how to access the other pages, but was able to download the whole book and sees things properly off-line.

I'd really like to see that latest version of the site is there have been updates since 2011.

On a related subject why when I access

http://museumofgaminghistory.org

don't I see links to the things we have been calling the MOGH? The link seems to be old stuff from the days of Chequers magazine. I would expect to at least see a link to The Chip Guide on that page.
