Looks like V5763, but the descriptions in TCR are cryptic because of limited space for text. Only TCR author who entered them knows for sure grin .

Some, but not all, are illustrated in The Chip Guide in the MOGH. Colors are inconsistent in the scans because they were done by different people. Your chip is not pictured. The TCR V5763 catalog number is assigned to a picture there, but in my opinion, it is not correct.

I've only been able to find seven varieties of this chip. TCR lists 10, which may all exist somewhere. Two are likely going to be very expensive... in the thousand$+ range.
