The problem for me is what to do when our guide books conflict (see Jim's post below) and I have no personal knowledge of the origin of the chip. I was hoping by posting it here first that I could get firm id then submit it. I don't know what the protocol is for a chip like this - submit it with the conflicting information or just don't submit it until better info is available.

I did not mean to be critical of the Chip Guide - it is a marvelous work. I support it 100%. Thanks to all those who work so hard to make it better every day.
