I'm no expert and I'm not certain how the inlays are applied to a chip but, It appears that some sort of backing used to handle the inlay remained on it when the chip was put under pressure. The grain of the inlay and rest of the chip are perfectly alined. It looks to me to be a genuine chip with a with a manufacturing error of some sort. The fact that the inlay did not seat well( some of it pulled off) is puzzling. No doubt someone familiar with the chip making process could tell what happened at a glance.
