The mold is made by Blue Chip Company, run by Mike Endy (the Endy family, formerly associated with Paul-son).
Located in Las Vegas.

In the last paragraph, it mentions this mold which the chip community usually refers to as "Shells" (or "Seashells").
However, Mike Endy is quoted as saying the actual mold name is "Six Flames".
