a mis-aligned 'registration' or that the 'registration' is off!
For the type of printing used for that inlay, there are several passes of the different inks being applied. in this instance, the blue was placed in the wrong location!
It is a common printing mistake and most printshops will try not to let mistakes like that out. However, for inlay printing, there was at least one sheet of inlays that share that same problem, and perhaps more. And while the printer may have tried to capture all of them and re-adjust the blue set-up, it is obvious that at least one poorly registered inlay made through the system!
