I have never seen this before so I have absolutely no idea how it can happen and what it can be worth. If someone knows I would like to know both how it happens and what it is worth.

Notice the bottom scan. The diamond design insert is missing at 4 o'clock.

Also I do not see it listed in my Chip Rack.
