Normally, these would be spotted and pulled from an order, and if they are missed, employees (cage, pit, etc.) often pull them. It is not an un-common error, but many are rather dramatic and there is a collector base for such errors. Here are several other examples from my meager collection of error inlays...

In the bigger scheme of such things, not many of these survive, which may be the collector attraction fot some.
