FYI, there's a fourth Carlino & Carlino / Fremont Coin chip -- $5 blue with 4-1/4" light blue inserts. OOR full graphic inlay with picture of the store manager on it.

I have all 4 (un-notched) that I purchased from them in 1997. At that time, they had a small coin store on Fremont Street. Located on the Fitzgerald's side of the street, in the block below Fitzgerald's. Not there anymore. They had one of those rotating cases with chips in it, but otherwise, it was a coin & jewelry store.

I acquired the chips at face value from them at the time. May be a mention of the chips in one of the 1997 issues of the club magazine, though I haven't checked.
