Given the CHIPCO manufacturing process...

there should be more of these out there, unless quality control spotted the error and pulled the others. (Not likely.)

CHIPCO, in a dye-sublimation process, prints a chip's image (in reverse) on a special carrier sheet. The sheet is placed over blank chips and then, using heat and pressure, to transfer ink to the surface of the blank chips. Shown here are 2 used, carrier sheets for 2 different chip issues, used to make 24 chips (one side) all at once. I do not know if both sides of a chip design is done at the same time, or if doing each side is a separate pressing. (Anyone know?)
