Is what I received after I tapped the send button where I had also explained the images will be on the chip board. Some of the fields are missing after the return as well as what I typed and the subject has explained above I have no idea what you are getting at.

This one attempt was much easier for me than others when I have to retype everything and not knowing if the form was sent and that is why I take a screenshot and post it here while sometimes only a portion is seen.
