Some of the chips were issued per the proofs but many weren't.
There was an issue with Gaming requiring proof (ID, Drivers Licence etc.) that the people depicted on the chips were 18 (or maybe 21) at the time the pics were taken.

Another fact that many won't know (although Andy Hughes might remember Mike Decker telling us) is that when the Ozzy Osborne Hard Rock chips were first released the two 'dog' chips were originally going to be Jack and Kelly. Jack wasn't old enough so gaming turned them down and the dogs were made instead.
