Because of a very busy schedule with a project I am working on and a mis-communication with the form not being mailed with the last issue of the CCA journal it is finally up on the CCA's website. You can email the information for anyone you wish to nominate as long as you provide me with all the information the form requests. If they are going to print in in the next issue of the journal I will extend the deadline to accommodate the delay in printing.

So let’s get things rolling and nominate someone desrving.
