I don't have much to add to the discussion.
I've had the chip for a while. I don't remember anything about acquiring the chip.
The mold is definitely Dice-Cards with the symbols on the black plastic so it doesn't show up well.
It is a bit odd in that the Dice-Cards is less deeply incused that other DI chips. In addition the edges seem more square than a regular Bud Jones CIC chip. Finally there's what might be an injection hole on one side. (It's on the yellow/black border just to the right of the drill hole.)
I think it's not a fake or counterfeit chip (who drills those?) My guess is that it's some sort of prototype. Beyond that I'll defer to the experts. Here are scans of the front and back.
