Here's a set of 3 chips from Caesars Palace --

The left one is the "correct" one with 3 sets of inserts.

The middle one is the "incorrect order" error chip with 4 sets of inserts.

And the right one is a "double error" in that is has the 4 sets of inserts AND the inlay (one side only, as shown) has a $25 denomination on it. (Made at the factory, not a sticker or other post-factory manipulation.)

I think I've shown these before on the BB, but quite some time ago.
