Dean, It wouldn't surprise me if the Cal-Neva had the most issues.
I'm not saying that's the case, but that place has been around since before the 1930s.
Their earliest chips that I'm aware of were issued in 1937.
And to my knowledge continued to issue chips for longer than any other casino in the US.
I do know the Cal-Neva's casino operated continually for longer than any other casino in the Country.
