A private message pointed me in the right direction.

The mold has been used for prototypes at the Harold's Club in Reno, and possibly prototypes at other casinos, and H.C. Edwards (the owner of the mold) issued sample chips on this mold, all pictured below.

So far I have not heard or found anything about any other gambling establishment using this chip mold.
