I put on my glasses and looked it up.

According to this old newspaper entertainment review, one Dennis Lewis owned the Nashville West:

And you'll find some things on your favorite impresario, Lloyd Hhickey, too. Also, if you scroll down, a Pat Murphy, Iarry Freitas & Warren Sjothen Card Room, 5147 Madison Avenue. If huntin' is more your style, pardner, get yerself one of those 169β€œ Remington 870 Shotgun 2V Vent Rib Reg. 189.95. Watch the deer drop dead of laughter as you try to maneuver that 169-foot long barrel.

The second address of Nashville West, 6601 Watt Avenue in North Highlands, is now the Second Slavic Church. I'm guessing there had to be a building tear-down and rebuild up there.

PS: I'll save y'all some time on the 5147 Madison address - it was The Sticky Wicket, one of the entries on my Dark Side of the Moon Cardroom list - had a license but did not make it past 1999 or so. Address now seems to be a bunch of condos. You find a Sticky Wicket chip, I'd sure like to buy it!
