You can certainly get off center chips but they should be rejected well before that stage.
When the chips are pressed they are way bigger than the finished article.
They are then ground or lathed down to size. Can involve removing up to 5mm. If a chip was off centre in the lathe that could happen.
However, I'm wondering if that chip was made at USPC and started off as a plain chip. They used different processes which I am not so familiar with.
