Inlays are made of several thin layers bonded together, and this is done well before they are pressed into the chip.
So had the defect been present in the inlay, or a void in the clay (not really possible) then the inlay would have become distorted during pressing.
The fact only the top transparent layer is affected means it had to have happened after pressing.
