It varied. Often at the gaming tables, especially in Atlantic City, which did not allow for direct purchase from the cage.
In Nevada, it seems it was the policy of each casino -- some at the tables, some via the cage, and some both. Occasionally, a casino would hear from Nevada Gaming Control, if their sales activities were more like outright sales. Remember, chips are supposed to be used "for gaming purposes".
But not from the gift shop.
