It's from an older catalog they mailed me in 1995; page seen below.

As you can see, there are two holes in the plaque that have been touched up in the printing.

Most of the catalogs they sent me back then depict serial numbers 0000... which, I assumed, were the start of a run of serial numbers. I've seen several photos through the years in articles/books where they show those 0000.. pieces with actual holes and I assume are part of what they kept.

I do recall you mentioning in a post, here, that you saw the plaque in London; belonging to someone. I may have responded.

I can also see graphic on the bottom of the lunette which might be on the reverse, but have never been able to figure it out...
