Pam, Rich - Kiva Club and Kiva Bar/Casino right across the street might look a little confusing, but the club (1932-1934) and bar/casino (1940-1945) were several years apart, so it was probably not confusing to the people at the time. The club was within the hotel, and seems to have been without any signage of its own. Kiva Bar/Casino was its own business with a sign over the door. The cocktail napkin for Kiva that Roy posted does not have an address, but I'll bet it was from the bar/casino in the 40s.
I believe all of the info in John Yee's article is correct, based on two or three LV Review Journal articles in the 30s. It's difficult to access those articles, all in microfilm, but I've seen the citations: Kiva Club opens at the Apache Hotel, 6/27/1932, and "Kiva Club now Apache Night Club", 5/4/1934. These citations are made on index cards for the hotel.
