What makes it a "crest & seal" chip?

My basic question is what makes a chip a "crest & seal" chip instead of just a solid chip? Is it the lack of an outer mold? Lack of a recessed inlay? Combination of things?

Also, does a chip have to be non-denominated to be considered a crest & seal chip?

Legitimate questions here. Just wondering what the guidelines are since I know almost nothing about them in general.
