Qume was a printer manufacturer in the Bay Area when I worked in that area. This chip must have been designed by Reliable for their sales dept.
PMSC construction, Qume on one side, a blank center on the other side. This sample has a small hole near the top that might have been intended for a small key chain??
Qume is now gone, merged with ITT in 1978, a big company that made lots of things. See the news article below.

You may remember the Daisy-Wheel printers of the 1970's. Qume used to make one of those.
