Good Evening All

I've been sorting through a collection of chips I recently bought. This beautiful chip was included. I'd not seen it before.
So, in order to have an excuse to post its picture, Is anyone else familiar with this chip? (I know what it is, all the info is on the other side that I'll post later...)
