I think this is the chip that Kreg has in her USPC chip book as 2040506.0. At least the thread-mounting holes are in the same location; I don't have the color version of the book.

Listed as sent to Chicago in 1917 by USPC. Other references say it ended up at Lincoln Hotel, Stockton, CA.
