I would assume a single pour; possibly same composition (unless center was poured and cured first?), though, if so, not sure if pigment properties in either/both weakened what I assumed was not a bond. Appears to be exactly at joining if both colors which might suggest more than a single pour?

The plaque was in storage; stationary... and not a fall or came into contact with anything else.
