After years of wondering, I was recently informed of the history of this octagon and chip. In short, the Opera House had the octagon made, Gaming denied it, and they issued the chip in its stead. Opera House got the last laugh however, the octagons were drilled and sold as keychain gift shop pieces for $5.99!
